    Agriculture.

    Go to www.dairyco.org.uk for various employment related forms. Although these specifically relate to farming, there are various free template documents which may be generally useful, including an example disciplinary policy, an absence policy, written warning letters, redundancy latters and medical letters and forms.

    Disciplinary procedures (for employers)

    Flexible working

    There are BERR model forms for employers to use as follows: (i) Form FW(A): Flexible working application form, (ii) Form FW(D): Flexible working appeal form; and (iii) Form FW(G): Flexible working notice of withdrawal form.

    Policies

    A selection of "Policy Documents" (including a Dignity at Work Policy, an Email and Internet Policy; Disciplinary Policy and Procedures; Absence Policy; Mobile Phone Policy; Dress Code Policy etc) are offered at reasonable prices by a website at www.employmentpolicydocuments.co.uk.


    Redundancy

    A series of semi-official Redundancy related forms on the Insolvency Service website includes non-insolvency related forms.


    Statement of particulars of employment

    A contract of employment automatically comes into existence when an employee starts work, whether in writing or not. An employer is required by law to provide employees with a written statement setting out the "main particulars" of their terms of employment (see generally notes at Statement of particulars of employment ). In many cases this is all that is needed but in cases where more detail is needed, for example where there are restrictive covenants and/or other special rights/obligations, it will be sensible for both the employer and employee to ensure that there is a written contract specifiying the terms in detail.

    The Government provides a useful and free Official example of a written statement of employment particulars (also on their website there are BERR/Business Link back up notes to their example written statement of employment particulars which includes a link to a template form which can be completed on-line).


    Stress

    The Health & Safety Executive provides a useful, free of charge, downloadable HSE "example stress policy" document on its website.


    Tax

    A useful set of UK and US Tax forms from Frank Hirth, Accountants is available available for download from their website.


    Tribunal forms

    On-line versions of tribunal forms ET1 (employment tribunal claim form) and ET3 (employer's response) are available
    from the
    forms section of the employment tribunal service web-site (see notes at Procedure of Employment Tribunals/forms and documents/official forms ).

    TUPE

    Appendices to the free 2009 TUPE Guide "Preparing and transferring services in the public and non-profit sectors" promoted by the CIPD contain various useful sample letters and documents in connection with service provision changes (see Transfer of business or undertaking/service provision change ).


    Work Permit and Visa application forms
